The four icons beside GTA 6's wanted stars show what the police know about you: the vehicle they saw you in, your clothes, your face, and whether you are with a partner. Clear a lead and its icon goes out. Confirmed
How leads work
Each lit icon is a thing the police can match against what they see. In the showcase, jumping into a car the police had not seen cleared the vehicle icon and a patrol unit drove straight past. A full change of clothes clears the clothing lead; previews say an outfit wheel, opened from the weapon wheel, changes clothes and pulls on masks on the spot. Confirmed Previews differ on whether a haircut alone is enough, with at least one saying it is not. Reported
Splitting up
Because "partner" is a lead, separating Jason Duval and Lucia Caminos is an escape tool. Rob Nelson says that when the police know they are chasing a pair, splitting up reduces what they can match against, which turns the two-protagonist setup into a way out of trouble rather than only a story device. See Jason and Lucia.
Tracking the car
Previews also describe the police tracking the vehicle they last saw you in, so a stolen car is a lead until you swap it (Car theft and keys). Shooting out a pursuing cruiser's tyres can flip it.
Practical reading
Wear something forgettable to a job and keep a change of clothes in the car; the pre-order Vintage Vice City Pack and Ultimate Edition outfits give you a second wardrobe. Speculation See Customisation.

Questions
What do the four icons next to the stars mean?
What the police have on you: your vehicle, your clothes, your face, and whether you are with a partner. Clear a lead (swap cars, change outfit, split up) and its icon goes out.
Does changing clothes help you lose the police in GTA 6?
Yes. Your clothes are one of four things the police track, and a complete change of outfit clears that lead.
